New York State Public Health Law requires all children entering or attending seventh, eighth and twelfth grades to receive an adequate dose or doses of vaccine against meningococcal disease.
WINTER SPORTS INFORMATION:
The school doctor will conduct sports physicals for the WINTER sports season on:
- November 12 for Cohort B
- November 17 for Cohort A
Any student athlete who does not have an up to date physical on file with the nurse, completed on the NYS School Health Examination Form, must stop into the nurse's office to schedule an appointment time to be seen by the doctor and complete a pre-screening. The physical MUST be completed on the NYS School Health Examination Form to be approved for athletics. Contact the school nurse with any questions.

How long should my child stay home if sick with a fever?
Your child may return to school after being fever free for 24
hours without the use of fever reducing medication, such as

How long should my child stay home with pink eye?
Students are referred to their physician for diagnosis and
treatment. Once the treatment is started, your child may return
to school 24 hours later as long as there is improvement.CHS Nurse's Office -
FamilyID registration for Interscholastic sports
All athletes must be registered each athletic season on FamilyID. Once your athlete is registered you will receive an email advising you if your child has been approved, or not approved. If they are not approved, there will be an explanation. You don't need to call the nurse's office to see if your athlete has been approved or not, you will get an email.

If I need to pick my child up early, what should I do?
If you need to pick your child up early you should send him/her in with a note explaining why you need to pick him/her up & the time you need them. Your child should bring the note to the main office upon entering the building. He/she will be given a pass excusing him/her from class at the specified time. Your child will then meet you at the circle. In the event of an emergency, when you have not sent in a note, go to the main office. One of the secretaries will page your child out of class. The attendance will be adjusted accordingly by the end of the day or, at times, the next day.

How long should the student stay home with impetigo?
Students are to be excluded from school until under treatment for
24 hours.

How long should my child be home with mononucleosis?
Students should be excluded from school until private physician states that the child may return. Students must present a doctors note indicating any activity restriction upon return to school.

Can my child attend school with a rash?
Students with undiagnosed rashes should be excluded from school until diagnosed and treated by their own doctor.

If my child is late, what should I do?
If your student is going to be late send him/her in with a note explaining why. He/she will sign in either at the circle or the bridge. The attendance will be fixed accordingly by the end of the day or, at times, the next day.

What if my child needs the use of the elevator?
Your child needs to bring the nurse a note from his/her doctor explaining how long & why the elevator is needed, along with a $10.00 deposit for the key. When the key is returned, your child will get the $10.00 back.
